This full-time position requires a commitment of 40 hours per week, typically Monday through Friday, with occasional Saturday coverage to ensure continuity of care.

Work Environment

This position is onsite in New York, New York, within a dynamic home care setting. This unique environment allows for a personalized approach to patient care, as you will travel to patients' homes to provide services, ensuring they receive treatment in the comfort of their environment. Key Responsibilities

• Conduct comprehensive assessments, managing both the physical and psychological issues presented by patients.
• Identify and interpret clinical signs and symptoms, determining the need for further intervention.
• Develop, implement, and adjust personalized treatment plans based on patient assessments and ongoing evaluations.
• Monitor and evaluate the effectiveness of medications and treatments, adjusting plans as needed based on the patient's response.
• Prescribe immunizations and medications in adherence to established protocols and guidelines.
• Facilitate communication among healthcare providers, ensuring a coordinated approach to patient care.
• Maintain accurate and timely documentation of patient encounters and update Problem/Medication lists during each visit.
• Engage in patient education, providing information on disease management and preventative care strategies.

Patient Population / Client Focus

As a Nurse Practitioner with Wide Stream Pain Solutions, you will work primarily with adults experiencing cardiovascular health issues. This includes individuals living with chronic heart conditions, patients undergoing rehabilitation, and those needing palliative care. Qualifications

• Master's degree in Nursing from an accredited Nurse Practitioner program.
• Current Registered Nurse (RN) License issued by the State of New York.
• Certification in Basic Life Support (BLS) to ensure patient safety.
• Exceptional interpersonal and communication skills, demonstrated ability to connect with diverse patient populations.

Preferred Experience

• A minimum of 2-3 years of clinical experience in cardiology or a related healthcare setting is preferred.

Compensation and Benefits
