Work Environment

Key Responsibilities

• Conduct comprehensive assessments by obtaining detailed patient histories and performing physical examinations relevant to cardiovascular health.
• Manage patient care by developing individualized care plans based on assessment findings, collaborating with cardiology physicians for optimal treatment strategies.
• Administer, supervise, and interpret various cardiac diagnostic tests such as stress tests and echocardiograms to ensure accurate diagnoses.
• Order and analyze laboratory tests, and integrate results into patient management plans to tailor therapies to individual needs.
• Educate patients and their families on essential cardiovascular health management, including lifestyle modifications and medication adherence.
• Prescribe necessary medications in accordance with state and federal regulations, ensuring that treatments align with best practices.
• Participate in team meetings and case discussions to promote coordinated and comprehensive care for patients with heart conditions.
• Stay abreast of the latest advancements in cardiovascular care through continuing education and training opportunities.

Patient Population / Client Focus

Our primary focus is on adult patients residing in an assisted living environment who are dealing with cardiovascular concerns and chronic illnesses. This role requires not only medical expertise but also a compassionate approach, as you will support patients who may be experiencing multiple health challenges and varying levels of independence.

Qualifications

• A Master of Science in Nursing (MSN) from an accredited institution.
• Completion of a Nurse Practitioner program approved by the New York State Education Department.
• Active New York State RN license and board certification through an accredited body (such as ANCC or AANP).
• Advanced Cardiac Life Support (ACLS) certification.
• Strong competence in Electronic Medical Records (EMR) systems and the Microsoft Office Suite.
• Excellent communication skills, both written and verbal, enabling effective interactions with patients and team members.

Preferred Experience

• A minimum of 2 years' experience working as a Nurse Practitioner, preferably in a cardiovascular care setting.
• Experience with geriatric patients and chronic disease management is highly desired.

Compensation and Benefits

This role offers a competitive salary ranging from USD 150,000 to 160,000 per year, commensurate with experience and qualifications.
