Key Responsibilities

• Conduct thorough patient assessments, including comprehensive physical examinations and detailed medical histories, as well as documentation of patient responses to treatment.
• Administer pain management interventions tailored to individual patient needs, including prescribing and monitoring medications, evaluating treatment efficacy, and modifying plans as necessary.
• Educate patients, families, and caregivers about pain management strategies, including the importance of medication adherence and lifestyle modifications that can enhance overall health.
• Collaborate closely with physicians, physical therapists, and other specialists to develop comprehensive treatment plans that align with each patient’s health goals.
• Utilize advanced medical technologies and systems to track patient progress and ensure seamless communication within the healthcare team.

Patient Population / Client Focus

The Nurse Practitioner will primarily service patients experiencing chronic pain conditions, including but not limited to neuropathic pain, joint issues, and post-operative pain management. The provider will play a vital role in improving the quality of life for these patients, focusing on holistic approaches that cater to both physical and emotional well-being.

Qualifications

• Master's degree in Nursing from an accredited institution.
• Current New York State licensure as a Nurse Practitioner (NP).
• Strong clinical skills and the ability to communicate effectively with diverse patient populations.

Preferred Experience

• A minimum of 2 years of experience specifically in pain management or a related field.
• Background in home healthcare or experience in a direct patient care setting.

Compensation and Benefits
