Work Environment

Situated in a vibrant area of Brookhaven, our outpatient office fosters a warm and collaborative atmosphere. Our team consists of healthcare professionals who share a commitment to providing empathetic care. The daily schedule typically runs from 9:00 a.m. to 6:00 p.m., with opportunities for evening clinics until 8:00 p.m. on occasion, as well as participation in weekend community outreach events and health fairs.

Key Responsibilities

• Conduct thorough health assessments and provide screenings for diverse individuals and families.
• Diagnose and manage a range of acute and chronic medical conditions, ensuring a focus on preventive care and health promotion.
• Work with local community organizations to develop and implement educational health programs aimed at increasing public health literacy.
• Deliver culturally sensitive care, advocating for patients' rights and access to necessary healthcare resources.
• Coordinate referrals and oversee follow-up care to maintain continuity in patients' treatment and health management.
• Maintain accurate medical records, ensuring compliance with all regulatory standards and ethical practices.
• Engage in community outreach efforts, participating in health fairs to conduct health screenings and disseminate vital health information.
• Stay abreast of developments in community health trends and continue to implement evidence-based practices to enhance patient care.

Patient Population / Client Focus

This role primarily focuses on serving the diverse populations of Brookhaven, including families and individuals from various backgrounds. As a Nurse Practitioner, you will address health concerns that range from preventive care to chronic disease management. Your efforts will contribute significantly to empowering community members through education and providing equitable healthcare, particularly to underserved populations.

Qualifications

• Current certification as a Nurse Practitioner.
• An active New York state nursing license.
• A Bachelor's degree in nursing; a Master's degree from an accredited program is preferred.
• Exceptional interpersonal and communication skills, with a demonstrated ability to engage effectively with diverse patient populations.

• A minimum of 3-5 years of clinical experience, ideally in community health or public health settings.

Compensation and Benefits
