Key Responsibilities

• Conduct thorough and comprehensive initial screenings, including patient history assessments and physical examinations, to establish a baseline for ongoing care.
• Maintain meticulous and up-to-date patient records, verifying accuracy and completeness with each visit to ensure compliance with healthcare regulations and standards.
• Analyze various diagnostic tests and data, interpreting results to make informed decisions regarding treatment plans and follow-up appointments tailored to individual patient needs.
• Deliver effective and compassionate care while adhering to productivity metrics, promoting high standards in patient service and health outcomes.
• Act as a key resource for patients by providing ongoing counseling, education, and guidance, particularly for those managing chronic illnesses or critical conditions.
• Educate patients on health-promoting practices, disease prevention, and management strategies, emphasizing harm reduction and lifestyle modifications.

Patient Population / Client Focus

Qualifications

• A Master’s degree in Nursing from an accredited institution, demonstrating extensive knowledge and training in patient care.
• Valid New York State licensure as a Nurse Practitioner (NP) with a focus in family medicine.
• A minimum of two years of experience as a licensed Nurse Practitioner in a clinical setting, specifically in family practice.

Compensation and Benefits

We offer a competitive annual salary ranging from $150,000 to $160,000, commensurate with experience and qualifications.
