Experienced Family Nurse Practitioner - Home Care Focus

Position Overview

Evergreen Family Practice is looking for a dedicated and skilled Family Nurse Practitioner (FNP) to join our dynamic team in Huntington, New York. This full-time, onsite position offers a competitive salary range of USD 125,000 – 140,000 annually. In this role, you will be instrumental in delivering high-quality healthcare in the home care setting, focusing on comprehensive patient assessment, diagnosis, and treatment of a diverse population.

Work Environment

As part of our onsite home care facility in Huntington, you will work closely with a multidisciplinary team of healthcare professionals, including X-Ray Technologists, Nurses, and Front Office Specialists. Key Responsibilities

• Supervise and lead clinic staff to uphold high standards of patient care, guiding fellow team members in their interactions with patients.
• Maintain timely and accurate patient charts, ensuring appropriate documentation and forwarding as needed to support continuity of care.
• Implement clinical protocols as developed with the Chief Medical Officer and Supervising Physicians, ensuring compliance with federal and state regulations.
• Order, conduct, and interpret diagnostic tests; prescribe medications and educate patients on the management of acute and chronic conditions.
• Perform therapeutic procedures such as incision and drainage (I&D), splinting, suturing, infection management, and wound care.
• Evaluate patients’ physical findings and histories to develop accurate diagnoses, ensuring a thorough and empathetic approach to patient interaction.
• Foster a positive work environment through effective communication and collaboration with team members.
• Participate in additional tasks assigned by management that contribute to the overall functioning of the practice.

Patient Population / Client Focus

Our patient population encompasses a wide range of demographics, including families, children, and elderly individuals. You will address various health concerns, both acute and chronic, and provide holistic care tailored to each patient's specific needs. A strong emphasis will be placed on building relationships and trust with patients, promoting their overall well-being within a comforting home care environment.

Qualifications

• A Master's degree in Nursing with a specialization as a Family Nurse Practitioner is required.
• Active Family Nurse Practitioner License compliant with New York State regulations.
• Proficient in adhering to the organization's operational standards and protocols.
• Exceptional interpersonal skills to maintain friendly and effective relationships with patients, clients, and healthcare team members.
• Strong ability to consult with patients through virtual communication platforms, enhancing care accessibility.
• Capability to thrive in a fast-paced and autonomous environment while managing multiple responsibilities efficiently.

Preferred Experience

• Proven experience working as a Telehealth Clinician, enhancing service delivery through digital platforms.
• Previous experience in home healthcare or similar settings is highly desirable.
