This full-time on-site position plays a crucial role in delivering high-quality primary care to adult patients enrolled in Medicare, with a focus on keeping them healthy, independent, and out of hospital settings. The salary for this position ranges from USD 120,000 to 160,000 annually.

Work Environment

The Nurse Practitioner will work within an engaging outpatient setting within local schools, collaborating closely with a multidisciplinary care team to enhance health outcomes for our patients. This full-time position operates Monday through Friday, from 9:00 AM to 5:00 PM, providing a structured work schedule. Key Responsibilities

• Deliver exceptional primary care services, emphasizing a patient-centered approach.
• Conduct comprehensive office visits addressing both routine and acute health concerns, ensuring all patients receive thorough evaluations.
• Administer annual wellness visits and health risk assessments, employing a holistic view of the patient's health and meticulous documentation practices.
• Coordinate care effectively with other healthcare providers, specialists, testing facilities, and agencies to enhance patient outcomes.
• Take a leadership role in population health initiatives, such as implementing evidence-based screenings and preventive care measures.
• Assist with phone triage and outreach, ensuring timely communication and support for our patients.
• Educate patients about their health conditions, care plans, and treatment options to foster understanding and adherence.
• Participate in community promotional activities to raise awareness of our health services.
• Perform home visits when necessary to provide care to patients who may have difficulties accessing our facilities.
• Regularly report to the Medical Director and engage with the operational leadership team to align on organizational objectives.

Patient Population / Client Focus

The primary focus of our Nurse Practitioner is the older adult population enrolled in Medicare. Our team is committed to delivering compassionate care that acknowledges the unique health challenges faced by this demographic. Qualifications

• A Master's Degree in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• Current DEA license for prescribing medications.
• Demonstrated experience in primary care settings, including geriatrics, internal medicine, or family medicine.
• Ability to navigate HCC documentation, ICD-10 coding, and health risk assessment processes effectively.
• A strong affinity for teamwork, with the capacity to collaborate across various functions within the organization.

Preferred Experience

• Previous experience working with Medicare patients and familiarity with their specific health care needs.
• Hands-on experience in a multi-disciplinary care environment, showing initiative and leadership capabilities.
