We are seeking a dedicated and compassionate Full-Time Nurse Practitioner to join our team at Advanced Cardiology PC in Plattsburgh, New York. Work Environment

As a Nurse Practitioner, you will work onsite in our outpatient office, collaborating within an integrated care team. Our team consists of physicians, nurse practitioners or physician assistants, medical assistants, and other support staff such as registered nurses and care managers. This collaborative approach allows our providers to focus on delivering high-quality primary care and managing population health. We partner with a network of specialists and hospitals to ensure our patients have access to excellent acute care services when needed.

Key Responsibilities

• Deliver outstanding primary care services, conducting office visits for both routine check-ups and acute health issues.
• Conduct Annual Wellness Visits and thorough health risk assessments, emphasizing holistic health and meticulous documentation practices to support patient care.
• Coordinate care with other healthcare professionals, specialists, diagnostic testing facilities, and community agencies to enhance patient outcomes.
• Lead initiatives in population health management, ensuring that patients receive necessary screenings, such as breast cancer screenings, in a timely manner.
• Engage in phone triage and outreach, assisting the care team to effectively respond to patient needs.
• Educate patients on their health conditions, care plans, and treatment options, fostering an understanding that empowers them in their healthcare decisions.
• Participate in community promotional activities that strengthen your relationships with patients and raise awareness about our services.
• Conduct home visits as required, ensuring continuity of care for patients with mobility challenges.
• Perform any additional duties as assigned, reporting directly to the Medical Director and collaborating closely with operational leadership.

Patient Population / Client Focus

Our primary focus is on older adults, particularly those on Medicare, aiming to keep them healthy and active within their communities. Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active and non-probationary Nurse Practitioner license in the state of New York.
• Active DEA license for prescribing medications.
• A solid background in primary care settings such as internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC (Hierarchical Condition Categories) documentation practices, ICD-10 coding, and health risk assessments.

Preferred Experience

• Experience in a patient-centered medical home or accountable care organization.
• Proficiency with electronic health record (EHR) systems and telehealth platforms.
• Strong interpersonal skills and the ability to work effectively within a team-oriented environment.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
