Position Overview

Lakefalls Family Medicine & Pediatric Clinic is actively seeking a dedicated and skilled Nurse Practitioner (NP) specializing in Cardiology to join our dynamic team in Plattsburgh, New York. This full-time, onsite position entails working within the outpatient practice of the Center for Advanced Cardiac Care (CACC), under the guidance of the Chief Nurse Practitioner and the Medical Director. Work Environment

Located in the picturesque Plattsburgh, NY, our clinic specializes in both family medicine and pediatrics, blending advanced healthcare solutions with a warm community atmosphere. The facility emphasizes a collaborative approach, where Nurse Practitioners work closely with Cardiologists, Registered Nurses, and a variety of healthcare specialists. The full-time schedule facilitates a balanced work-life dynamic, as you will operate from Monday to Friday, 9 AM to 5 PM, with no on-call duties, nights, or weekends.

Key Responsibilities

• Collaborate with Cardiologists and healthcare professionals to manage and oversee patient care.
• Perform comprehensive history-taking and physical examinations on cardiology patients.
• Manage chronic conditions such as hypertension and high cholesterol, providing evidence-based treatment plans.
• Administer outpatient Electrophysiology services, including the management of arrhythmias.
• Evaluate and treat acute conditions like Coronary Artery Disease, Congestive Heart Failure, and Stroke/Transient Ischemic Attack.
• Conduct and interpret stress tests as part of patient assessments.
• Prescribe medications, making informed decisions based on each patient’s unique healthcare needs.
• Refer patients to specialists within our network when necessary to ensure comprehensive care.
• Provide education to patients and their families about cardiac conditions and preventative health strategies.
• Utilize electronic health record systems for documentation and coordination of patient care.

Patient Population / Client Focus

The NP role at Lakefalls Family Medicine & Pediatric Clinic primarily serves a diverse patient population, focusing on both adult and pediatric individuals with cardiovascular concerns. Patients may range from those with chronic conditions requiring ongoing management to acute cases needing immediate intervention. Qualifications

• Successful completion of an accredited nursing program and a Bachelor’s degree in Nursing.
• A Master’s or Doctoral degree in Nursing, specifically from a Nurse Practitioner program.
• Active and unrestricted Nurse Practitioner license in the state of New York.
• Certification relevant to the field of Cardiology is preferred.
• Strong interpersonal skills and a commitment to providing compassionate patient care.

Preferred Experience

• Experience in outpatient Cardiology, particularly in Electrophysiology and arrhythmia management.
• Prior clinical exposure managing chronic conditions such as heart disease and diabetes.
• Familiarity with stress testing and interpreting cardiovascular diagnostic procedures.
• Ability to thrive in a collaborative, multidisciplinary team environment.

Compensation and Benefits
