Work Environment

This position is primarily onsite, allowing you to engage directly with patients in their homes and at our office in Plattsburgh. Key Responsibilities

• Conduct thorough assessments and evaluations of patients’ health status, developing evidence-based treatment plans that meet their individual needs.
• Utilize standardized protocols to diagnose, treat, and manage acute and chronic health conditions, adjusting care strategies as needed based on patient progress.
• Educate patients and their families about health maintenance, chronic disease management, and preventive care practices, empowering them to make informed decisions.
• Coordinate care with a multidisciplinary team of healthcare providers to ensure comprehensive management of patient needs.
• Document patient interactions clearly and accurately in electronic health records (EHR) to maintain continuity of care.

Qualifications

• Must possess a Master’s degree in Nursing from an accredited institution.

Compensation and Benefits
