Responsibilities Promote Elara Caring’s philosophy, mission statement, and administrative policies to ensure quality of care. Evaluate patients to determine rehabilitation needs and potential for achievement, then develop a plan of care in accordance with findings. Admit patients and assess speech‑language therapy needs according to OASIS and PPS regulatory guidelines. Conduct the initial therapy evaluation visit and re‑evaluate patients’ speech‑language therapy needs during each visit. Perform follow‑up assessments according to OASIS and PPS regulatory guidelines. Initiate a patient safety plan using the patient, family, and community resources. Communicate significant findings, problems, and changes in condition or environment to the appropriate supervisor, physician, and other team members involved with patient care. Participate in the implementation and development of the plan of care to ensure quality and continuity of care and proper discharge planning. Verify the plan of care prior to each visit and provide care according to physician’s orders, assessment data, and established standards and guidelines. Write physician orders to cover additional visits and changes to the plan of care, per agency policy. Incorporate patient‑care goals established in the plan of care into therapy care, as evidenced by documentation in therapy notes. Initiate and revise the plan of care in response to identified patient‑care issues. Perform direct speech‑language pathology services in accordance with accepted standards of practice and certified by the patient’s physician. Select diagnostic and therapeutic materials appropriate to patient needs. Instruct patients and/or their caregivers in the home therapy program, providing modification and clarification when necessary. Ensure clinical notes indicate continuing communication and coordination of services with physician, other staff, and disciplines. Ensure that all therapy notes, supply logs, and visit logs are accurate and turned in to the office according to office policy. Work in a physically demanding, high‑stress environment that includes full range of motion, patient handling, lifting, and transfer. Potentially work irregular hours, including call hours if applicable. What Is Required Master’s degree from an accredited educational program in Speech‑Language Pathology. Current, unrestricted SLP license in the state of work. Minimum one (1) year of experience as an SLP in a clinical care setting. Able and willing to travel within branch/office coverage area. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Willing to travel up to 50% of the time. Salary range: $78,000 – $110,000 annually, with potential additional earnings for work beyond quota, bonuses, on‑call, or other performance‑based compensation variables.
