Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments, crafting individualized treatment plans to facilitate recovery.
• Deliver physical therapy interventions that focus on enhancing mobility, strength, balance, and overall functionality.
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes.
• Educate patients and their families on therapeutic goals, discharge protocols, and exercises to continue their journey at home.
• Document evaluations, treatments, and patient progress in adherence to facility regulations and guidelines.
• Engage in care team rounds and interdisciplinary meetings to promote coordinated and holistic patient care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited educational institution.

Certifications

• Current state licensure as a Physical Therapist .
• CPR certification preferred.
• Specialty certifications (such as neurological or geriatric) are a plus but not required.

Skills

• Previous hands-on experience is strongly preferred.
