About the Position

Internal Medicine / Family Medicine Locums - SNF/LTC | Montana

Locums coverage is needed in Montana for a geriatric skilled nursing facility/long-term care population. This is a 3-day-per-week, daytime opportunity with no call and dedicated APP support.

Highlights

• Tuesday-Thursday preferred
• 7:00 AM-5:00 PM , 10-hour onsite shifts
• No call - hospitalists cover overnight call
• Average 12-14 patients per shift
• Dedicated APP support with physician supervision
• Geriatric and medically complex patient population

Responsibilities

Provide comprehensive medical oversight for SNF rehab and LTC residents, including:

• SNF admissions and H&Ps
• Routine/chronic and acute care
• Regulatory and recertification visits
• Medication management
• Care planning and interdisciplinary collaboration
• Family/goals-of-care discussions
• Transitions of care and palliative/comfort-focused care
• Required documentation and regulatory compliance

Requirements

• MD or DO
• BC/BE in Internal Medicine or Family Medicine
• SNF, LTC, post-acute, transitional, or geriatric experience
• ACLS and BLS
• Montana license required; active IMLC LOQ accepted while awaiting licensure
