Job Description:

• Review and process referral packets to ensure all required information is complete and accurate before the patient's first visit.
• Enter referral and patient information into internal systems in a timely manner.
• Coordinate with referral sources, clinicians, and internal departments to obtain missing documentation and resolve discrepancies.
• Verify health insurance coverage requirements and confirm eligibility for services.
• Maintain accurate records and ensure compliance with agency policies and regulatory requirements.
• Schedule home health visits and support ongoing home health scheduling activities.
