Social Worker II: Diagnostic Assessment & Community Health

MetroHealth System in Cleveland Heights is seeking a licensed social worker to join the team at the Cleveland Heights Health Center. The role supports patient care by evaluating needs and coordinating social work interventions. The position requires a Master's Degree in Social Work and current Ohio LSW licensure, with the ability to engage patients across age groups from children to adults.
