Remote LTSS Care Manager - Travel Required
Molina Talent Acquisition
Molina Healthcare seeks a dedicated care coordination professional to support LTSS services across Wisconsin. You will work with a multidisciplinary team to assess needs, develop and monitor care plans, and ensure integrated, quality, cost-effective member care.
Travel is 25–40% to accommodate in-person assessments and team collaboration. Required are health care experience (LTSS preferred) and strong communication skills.
