Charge Nurse - LVN/RN at Cedar Bayou

Charge Nurse – Focused Post Acute Care Partners (FPACP), Cedar Bayou, Baytown, TX 77521 Position Summary Provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice, regulations, and directives from the Director of Nursing. Lead or direct licensed and non‑professional staff in the delivery of direct resident care and support functions. Essential Functions Provide nursing care, plan, organize, and direct nursing functions for patients in the unit. Ensure resident safety and maintain acceptable standards of patient care. Identify problems and guide personnel to appropriate solutions. Create a working climate that promotes growth and job satisfaction for personnel. Accurately and promptly implement physicians’ orders. Support and enforce infection‑control policies and procedures. Maintain a professional approach with confidentiality. Cooperate and maintain good rapport with nursing staff, medical staff, other departments, and visitors. Obtain and document nursing history from the patient and/or significant other that is comprehensive of the present condition and prior problems. Perform basic assessment of the patient, demonstrating adequate knowledge of anatomy, physiology, and pathology. Record vital signs, note changes, and pursue further investigation as needed. Assess for and notify physician and other appropriate parties of changes in condition. Use assessment information to develop a care plan before the end of duty time, ensuring information is available for incoming personnel. Use care plans as a basis for providing safe and therapeutic care to patients. Consistently follow established standards, policies, and procedures in providing nursing care. Accept accountability for clinical care of assigned patients, including supervision of non‑professional personnel. Communicate with the patient, family, and other members of the health care team to achieve the goals of the care plan. Perform technical skills considered basic to the type of care given in the unit. Respond appropriately in urgent and/or emergency situations. Support standards of nursing care through adherence to existing policies and procedures. Seek guidance when encountering nursing procedures and responsibilities that have not been mastered. Promote positive public relations with patients, residents, family members, and guests. Use tactful, appropriate communication in sensitive and emotional situations. Adhere to dress code and appropriate workplace behavior at all times. Utilize electronic medical record charting (point‑click care). Qualifications Current LVN/RN license. CPR certification required. Ability to understand, remember, and carry out verbal or written instructions in English. Ability to complete assignments in a timely manner. Ability to be flexible and positive with intermittent interruptions. Current LVN license (duplicate stated in responsibilities; retained for clarity). Physical Requirements Ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching, and lifting up to 50 pounds, with or without reasonable accommodations.
