Field Service Coordinator

Field Service Coordinator (Care Coach 1) assesses and evaluates members’ needs to achieve and/or maintain an optimal wellness state. The coordinator guides members/families toward and facilitates interaction with resources appropriate for their care and wellbeing. Work assignments are often straightforward and of moderate complexity. Responsibilities Administer ongoing long‑term services and support (LTSS) related assessments through person‑centered thinking approaches. Contact members both telephonically or in‑person to establish goals and priorities, evaluate resources, develop a plan of care, and identify LTSS providers and community partnerships. Develop and modify Service Plan and involve applicable members of the care team in care planning (e.g., Informal caregiver coach, PCP). Support members through navigation of their LTSS and related environmental and social needs; use available information about members to prevent duplicative assessments. Focus on supporting members or caregivers in accessing long‑term services and support, social, housing, educational and other services, regardless of funding sources to meet their needs. Assist members in maintaining Medicaid eligibility. Collaborate with Medical Director/Geriatrician/Care Coordinator as necessary to ensure cohesive, holistic service delivery and positive member outcomes. Required Qualifications Bachelor’s degree in health and human services field. 2 or more years of related experience. Patient‑facing role; subject to Humana’s TB screening program. Screening for TB required if selected. Passionate about contributing to an organization focused on improving consumer experiences. Valid state driver’s license and personal vehicle liability insurance meeting state minimum or $25,000 bodily injury per person, $25,000 bodily injury per event, $10,000 for property damage (or higher). Separate room with a locked door usable as a home office to ensure privacy while working. Must reside in the Indianapolis IN metro area. Preferred Qualifications Previous experience with Medicare & Medicaid recipients. Electronic case‑note documentation experience in multiple computer applications. Experience with health promotion, coaching, and wellness. Knowledge of community health and social service agencies and other community resources. Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. Scheduled Weekly Hours: 40 hours per week. Pay Range: $53,700 - $72,600 per year.
