Physical Therapist or Physical Therapist Assistant

Role: Physical Therapist Assistant supporting seniors in skilled nursing and post-acute care settings. Responsibilities Implement physical therapy treatment plans tailored to individual patient needs. Assist with therapeutic exercises and functional mobility training to enhance strength, balance, and independence. Document patient progress accurately and thoroughly. Collaborate closely with Physical Therapists, nursing staff, and families to ensure coordinated care. Maintain patient safety and dignity throughout all interventions. Qualifications & Skills Compassionate and patient-centered approach to care. Ability to work effectively within a collaborative, multidisciplinary team.
