Seven Healthcare are seeking an experienced RN specializing in Case Management for a travel assignment in Savannah, GA. This role involves working closely with healthcare providers to implement comprehensive care plans and ensure optimal patient outcomes. The position requires travel across Chatham, Liberty, Bryan, and Effingham counties.

- Collaborate with primary care providers to execute comprehensive care plans.
- Identify and reassess cost-efficient, appropriate care levels.
- Implement non-medical support to ensure treatment plan compliance.
- Coordinate care transitions as necessary.
- Work with physicians and other professionals to transition patients appropriately.
- Engage specialty resources to achieve optimal resolutions.
- Maintain detailed records of outcomes during the management process.
- Identify opportunities for health promotion and illness prevention.
- Act as a patient advocate by safeguarding privacy and confidentiality.
- Current, unrestricted GA RN license.
- Associate or Bachelor's Degree in Nursing or related field.
- Proficiency with OASIS documentation and knowledge of HCHB.
- Experience in home health settings.
- Case Management certification.
- Ability to analyze complex medical information and make rational decisions.
