Remote Member Advocate & Nursing Home Liaison

American Health Plans in Topeka, KS is seeking a Member Advocate to provide exceptional, customer-focused support across local service areas. This is a work-from-home role with travel to partner nursing homes within a defined region and no fixed office space. You will welcome new members, conduct monthly visits, cultivate relationships with network providers and nursing facility staff, sponsor group activities, document all activities, and ensure CMS compliance while coordinating with Plan #J-18808-Ljbffr
