Responsible for the overall process of resident assessments and care planning and weekly resident care conferences for the appropriate healthcare unit. The Resident Care Coordinator must demonstrate an awareness that the needs of the residents and family members are first and foremost.SPECIFIC DUTIES AND RESPONSIBILITIES:Assist the Registered Nurse Assessment Coordinator with the MDS/RAI program including the admission, quarterly, annual and change in status assessments and complete required updates with insurance companies as needed.Assist practitioners with wound rounds while managing subsequent orders, care plans, and family communications.Oversee the Restorative Nursing Program, to include program compliance, documented quarterly reviews, weekly meetings, and resident/family communication.Participate in and/or lead as appropriate, the care plan meetings on the units along with the Interdisciplinary Team.Make changes to and update the resident care plan based on physician orders, observations, resident's response to treatment and data collected from all available sources.Communicate with physicians, including primary care physicians and specialists, as well as families and other responsible persons as appropriate.Monitor the administration of medications to residents according to the facility policy, including such things as appropriate storage, timeliness, and accuracy of dispensing, discarding and documentation.Complete the Resident Census and Condition of Residents form and the census roster, or other mandated reporting forms.Participate in interviews and hiring professional and non-licensed staff assigned to respective healthcare unit, keeping in mind the needs of the whole organization.Assist in orientation and training of staff.Supervise nurses and certified nurse aides in healthcare unit providing leadership, training, and coaching to the nurse and certified nurse aide on each shift and ensure that their job is being performed to meet the high standards of the Community at Rockhill and our residents.Evaluate staff performance as scheduled using accepted tools; elicit input from nursing supervisor for performance evaluations for staff who work on their respective shifts.Counsel staff on disciplinary issues including performance, attendance, punctuality, and adherence to their job description and the adherence to policies and procedures.Carry out an investigation of suspected abuse, neglect, or other patient care issues in a timely and thorough manner and that the details are reported to administration immediately and documented.Responsible to review the accident and incident reports. Monitor reports for trends and for potential changes in procedure and policy to prevent future problems. Incorporate recommendations about resident care in the individual resident's care plan immediately. Assure that information is communicated to all caregivers within a reasonable amount of time.Participate in development and evaluation of new programs, policies and procedures required for accreditation or licensure.Make recommendations to the Director of Nursing during the budgetary process and in clinical decision-making that impact the financial health of the organization.Partner with the DON to develop knowledge of various DON responsibilities, reporting, and regulatory standards and assist with DON coverage/responsibilities in their absence.Share on-call responsibilities, participate in the on-call schedule, and assume on-call duties.Perform other duties as directed by DON.EOE STATEMENT: We are an Equal Opportunity Employer.
