Duties & Responsibilities Patient Care Develop, initiate, and oversee the patient’s plan of care, including interdisciplinary assessment and individualized care planning in partnership with the patient, representative (if any), and caregiver(s). Initiate proper preventative and rehabilitative nursing procedures and provide services as ordered by the physician according to the plan of care. Provide patient, caregiver, and family counseling and education. Prepare clinical notes and communicate with the prescribing physician and other healthcare practitioners regarding the current care plan. Identify discharge‑planning needs and implement them prior to patient discharge. Communication Keep clinical notes up‑to‑date and update the primary physician at least every sixty days. Communicate patient needs and report any changes in the patient’s condition to the physician; acquire physician orders as required. Coordinate care with community health related persons to support the care plan. Additional Duties Participate in the HHA’s quality assessment and performance improvement program and sponsor in‑service training. Serve on the on‑call duty roster as defined by the on‑call policy. Ensure arrangements for equipment and other necessary items and services are available. Instruct, supervise, and evaluate home health aide care provided every two (2) weeks. Supervise Licensed Practical Nurses (LPNs), Licensed Vocational Nurses (LVNs), Home Health Aides (HHAs), and Certified Nursing Assistants (CNAs) as directed. Requirements Graduate from an approved school of nursing and hold a valid nursing license in the state of practice. Minimum of two years of experience, at least one of which is in public health or home‑care nursing (preferred). Maintain current CPR certification. Hold a valid, insured driver’s license and have an automobile in good working order. Be self‑directed and able to work with minimal supervision. Demonstrate excellent observation, problem‑solving, verbal and written communication skills, and nursing competency. Show ability to organize and prioritize workload independently. Ability to walk, stand, lift, position, or transfer patients, and lift supplies and equipment; acceptable reach, stoop, bend, kneel or crouch; adequate visual and hearing acuity.
