Clinical Liaison Job Summary Join our dedicated team at White Oak Rehabilitation and Nursing at Divine, a leader in the rehabilitation industry, as a Clinical Liaison. We are seeking a highly skilled and compassionate professional to act as a liaison between our healthcare facility and the community, ensuring seamless delivery of care to our patients. As a Clinical Liaison, you will be responsible for facilitating communication between patients, families, and healthcare providers to promote optimal outcomes and satisfaction. Responsibilities Collaborate with patients, families, and healthcare providers to develop and implement personalized care plans Conduct thorough assessments to identify patient needs and develop strategies for optimal care Serve as a liaison between our facility and other healthcare providers, ensuring timely and effective communication Provide education and support to patients and families to empower them to take an active role in their care Identify and address any barriers to patient care, such as social, emotional, or financial challenges Maintain accurate and up-to-date records of patient interactions and care plans Participate in quality improvement initiatives to enhance patient outcomes and satisfaction Requirements At a minimum of 2 years of experience in a long term skilled nursing facility setting What We Offer We offer excellent pay based upon experience, PTO and benefits #J-18808-Ljbffr
