Senior Nurse Case Manager: Care Coordination Leader

Blue Shield of CA's Care Management team coordinates, educates, and advocates care for the Dual‑Special Needs (DSNP) population. The Case Management - Nurse, Senior will report to the Manager of Care Management within Medical Care Solutions and will manage a caseload of DSNP members, review Health Risk Assessments, and complete Individualized Care Plans.
