Case Management Care Coordinator

Position Summary LAUKOA is seeking full‑time Case Management Care Coordinators to support the California Army National Guard (CA ARNG). This position plays a critical role in maintaining Service Member medical and dental readiness by managing cases, tracking referrals, documentation, and medical claims. Coordinators work closely with case managers, unit representatives, and readiness personnel to ensure accurate and timely processing of health information. This is a full‑time role supporting both daily operations and weekend Medical Readiness Events (MREs) as needed. LAUKOA delivers health readiness services that meet the deployment requirements of the CA ARNG. This position is Service Contract Act and is subject to Health & Welfare benefits. Qualifications Degree/Education: Associate degree in human services or related field, OR equivalent work experience Experience: Minimum of 2 years’ experience working with individuals experiencing medical or dental conditions Strong customer service and interpersonal communication skills Familiarity with common clinical assessment tools and documentation standards Demonstrated ability to work independently and as part of a team Comfortable interacting with diverse and occasionally difficult populations Ability to maintain HIPAA standards and handle sensitive medical information appropriately Background Must be a U.S. citizen or lawful permanent resident with work authorization. Must be able to pass background check for access to government systems and CAC issuance. Preferred Working knowledge of AR 40-501, AR 40-502, DA PAM 40-502, and other IMR and deployment‑related guidance Prior experience in military medical/dental readiness or administrative processing Familiarity with military culture (e.g., veteran, military family member, or volunteer) Experience supporting mobile or field‑based medical clinics Proficiency in AHLTA/MHS Genesis, and other DoD systems Proficiency with military medical administrative systems, including MEDCHART and MODS Additional Requirements May require some travel throughout California Occasionally required to stand or walk during MREs Continually required to sit, concentrate, and use a computer Must be able to lift/carry medical records or screening equipment occasionally We offer a best‑in‑class benefits program including medical, dental, and vision insurance; a 401(k) program with employer match; paid vacation and sick leave; employer‑paid basic life and AD&D insurance; an Employee Assistance Program; and a flexible work environment.
