Key Responsibilities\n

• Conduct and document thorough biopsychosocial assessments to identify the mental health, substance use, and medical needs of patients. This includes utilizing electronic health record systems for documentation and progress tracking.

• Provide individual, couples, and family counseling to address a wide range of behavioral health issues, including crises intervention and support for those experiencing severe mental health challenges.

• Facilitate group therapy sessions that promote healing and skill-building among participants, fostering a supportive environment for shared experiences and learning.

• Liaise with healthcare providers and community resources to ensure integrated care approaches are taken, including referring patients to medication assistance programs and additional community support services.

• Engage in regular team meetings focused on quality improvement processes, sharing insights and data to enhance service delivery and patient care.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution, with an active LCSW license in the state of California.

Compensation and Benefits\n
