Physical Therapist (PT) – Job Description

Job Purpose: Evaluate patients, develop individualized care plans, and provide or direct physical therapy interventions prescribed by the referring physician, while considering the patient's physical condition and age-related needs.

Key Responsibilities:

• Evaluate patients to determine physical therapy needs.
• Develop individualized treatment plans and goals.
• Provide skilled physical therapy treatments and therapeutic interventions.
• Administer or direct appropriate physical agents and rehabilitation procedures.
• Monitor patient progress and modify treatment plans as needed.
• Provide therapy appropriate to the patient's age and physical condition.
• Work with physicians and other healthcare professionals as part of the rehabilitation team.
• Maintain appropriate patient documentation and treatment records.
• Provide rehabilitation services to patients in a geriatric and rehab population, depending on the specific assignment.

Required Qualifications:

• Bachelor of Science in Physical Therapy
• Active New York State Physical Therapist (PT) license
• Master's degree in Physical Therapy preferred
• CSCS (Certified Strength & Conditioning Specialist) preferred/required on the current Staff PT posting
• Prior experience is not required for the current Staff PT position; new graduates may be accepted with up to six months of on-the-job training.
• Previous experience in a rehabilitative medicine setting is preferred.
