Compensation: $95,000 - $130,000 per year plus incentive visit pay

Location: Greater Twin Cities Metro Area (21-county service area)

Schedule: Full-Time with On-Call Rotation

What You'll Do
•Step into different territories to support caseload coverage and census needs .
•Provide skilled nursing care, assessments, and care planning .
•Complete OASIS visits and coordinate interdisciplinary care .
•Educate patients and families on care plans and recovery goals .
•Collaborate with physicians, therapists, and care teams.
•Travel throughout the metro while maintaining flexibility and independence .

What We're Looking For
•Active Minnesota Registered Nurse (RN) license.
•Strong clinical judgment and ability to adapt quickly.
•Comfortable working independently in a field-based, travel-focused role .
•Experience in home health preferred, but we will train the right RN.
•Reliable transportation and willingness to drive throughout the metro.
