Key Responsibilities\n

• Conduct thorough psychosocial assessments for patients and families to identify emotional, social, and environmental strengths and challenges related to their health status and treatment plans.

• Develop, implement, and oversee individualized care plans, employing appropriate social work interventions to enhance patient coping strategies.

• Provide crisis intervention and therapeutic sessions including individual, group, and family therapies as needed, ensuring a therapeutic environment that fosters recovery.

• Collaborate closely with healthcare teams, including physicians and nurses, to develop and execute effective discharge plans, facilitating smooth transitions for patients post-hospitalization.

• Educate and empower patients and their families regarding coping mechanisms and adjustments related to life-altering diagnoses and illnesses.

• Participate in on-call rotations to address urgent patient needs and facilitate after-hours support.

• Perform clinically necessary evaluations to assess patients for mental health conditions such as depression and anxiety, particularly in oncology settings, and provide short-term counseling when required.

• Present educational materials and participate in conferences to share expertise in psychosocial oncology practices with other health professionals.

• Access community resources, creating linkages for ongoing support for patients, particularly cancer survivors, to enhance their quality of life.

• Utilize knowledge gained through research and volunteering to better inform practices and improve community services available to patients.

The LCSW will primarily work with patients facing chronic and life-threatening illnesses, including cancer patients in hospice care. Qualifications\n

The ideal candidate will possess a Master’s Degree in Social Work from an accredited institution, alongside a valid LCSW license in the state of Texas. Experience in healthcare settings, particularly hospice or palliative care, will be highly regarded. Preferred Experience\n

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 100,000 per year, commensurate with experience and qualifications.
