Full‑Time Nurse Practitioner - Kokomo, IN AppleGate Recovery, a division of BayMark Health Services, is a multi‑site, multi‑service organization that provides opioid use disorder treatment and rehabilitation across the country with a patient‑centered approach. We combine evidence‑based medication‑assisted treatment with counseling and behavioral services to offer the most successful options for many patients. Description Full‑time Nurse Practitioner will provide direct care to patients seeking recovery from opioid use disorder. The role involves comprehensive assessment, treatment planning, and ongoing clinical support in an outpatient clinic setting. Essential Roles and Responsibilities Demonstrate knowledge of substances of abuse, addiction disease, and detoxification signs and symptoms. Collect and record complete patient histories, assessments, and treatment plans in a systematic, accurate manner. Provide counseling, education on medication‑self‑administration, and health education as needed. Document patient progress or regression following established policies during appointments or when notified of status changes. Supervise and instruct patients in oral medication administration and health education when required. Adhere to program policies and procedures. Qualifications | Education | Certifications Licensed nurse practitioner in the state of Indiana. Minimum six months of MAT/Addiction experience. Passed drug screen and criminal background check. Certified by the American Association of Nurse Practitioners (AANP).
