Position Overview

Dynamic In-Home Care is seeking a Full-Time In-Office Liaison / Intake Coordinator to join our growing team.This position serves as an important connection between patients, families, referral sources, healthcare professionals, and Dynamic's internal care teams. The ideal candidate is personable, organized, responsive, and comfortable working in a fast-paced healthcare environment.

The In-Office Liaison / Intake Coordinator will help coordinate new patient referrals, communicate with referral partners, assist with the intake process, and ensure patients and families have a smooth transition into care.

Key Responsibilities

• Serve as a primary in-office point of contact for patients, families, referral sources, and healthcare professionals.
• Receive, coordinate, and process new patient referrals and intake information.
• Communicate with hospitals, physicians, medical groups, discharge planners, case managers, social workers, and other referral partners.
• Gather and accurately enter patient demographic, clinical, insurance, and referral information into the company's EMR and other systems.
• Coordinate patient introductory calls and help patients and families understand the next steps in starting services.
• Collaborate with the Intake, Transitional Care, Clinical, Scheduling, and Care Coordination teams to facilitate timely admissions.
• Follow up with referral sources and families regarding the status of referrals and admissions.
• Help ensure all necessary documentation and information is received prior to the start of care.
• Communicate patient needs and important updates to the appropriate internal team members.
• Assist with coordinating services and ensuring patients are connected with the appropriate Dynamic resources.
• Maintain professional and timely communication with referral partners and healthcare professionals.
• Help develop and maintain strong relationships with hospitals, physicians, healthcare organizations, and community partners.
• Identify opportunities to improve the patient and referral-source experience.
• Assist with administrative duties, data entry, reporting, and other operational responsibilities as needed.

Preferred Qualifications

• Previous experience in healthcare, home health, hospice, medical office, hospital, or healthcare administration.
• Experience working with hospitals, physicians, case managers, discharge planners, or referral sources.
• Previous intake, admissions, liaison, scheduling, or care coordination experience.
• Experience using an Electronic Medical Record (EMR) system.
• Knowledge of home health, caregiving, hospice, or other home-based healthcare services.
