Job Summary The Community Health Nurse is supervised by the Community Health Nurse Manager and works closely with the communities served by Neighborhood House across King County to promote entire family wellbeing, dignity, and health. This position supports the mission framework and strategic plan of Neighborhood House in both the Child Development (CD) department, and the Education and Community Services (ECS) departments. Within the CD department, the Community Health Nurse contributes to the school readiness and positive physical outcomes of low-income children in our Head Start and Early Head Start programs. Within the ECS department, the position contributes to client well-being by providing nursing assessments and case management services to elderly and/or disabled clients enrolled in our Medicaid waiver long term care programs. This position will be work at many sites including our Head Start and Early Head Start Neighborhood House locations in Seattle, Burien, Tukwila, and includes home visits to clients in King County. - Support Managers and Supervisors in determining whether the program is able to achieve compliance and quality benchmarks.
- Complete all required program reports accurately and in a timely manner.
- Anticipate future trends and articulate credible visions that enable innovative and appropriate program planning. Design, implement and evaluate program development strategies.
- Support staff in reviewing, analyzing, and utilizing the health services delivery data in ChildPlus for the purpose of planning health initiatives, training, and support for families.
- Review health records of enrolled children; determine whether children are up-to-date on a schedule of preventive and primary health care when a physician has not made this determination.
- Determine whether there is a need for further medical or dental follow-up for children and assist families in scheduling care.
- Assist the program in obtaining further diagnostic testing, examination, and treatment for individual children.
- If applicable, meet with or conduct a phone interview with parents regarding health findings and assist families in understanding medical information and the need to complete prescribed treatment plans.
- Use pertinent health records and parent communication to develop and implement Individualized Health Plans.
- Maintain awareness of current health services available in the community and work with physicians and other local health care providers to meet the needs of enrolled children.
- Complete site visits to observe children who have been referred for health concerns.
- Collaborate with Managers to ensure classroom and socialization modifications are in place to meet the needs of individual children.
- Provide and arrange for appropriate trainings for staff, including how to conduct hearing and vision screenings as needed.
- Provide training to staff to assist them in understanding how to include children with unique health needs in classroom or home-based activities.
- Plan and lead the Health Services Advisory Committee, scheduling regular meetings.
- Work collaboratively with the Registered Dietician, Mental Health Consultants, and other staff who support the delivery of comprehensive health services.
- Document all health/family services that have been provided in ChildPlus.
- Monitor health records via ChildPlus and support staff in conducting follow-up with families as needed.
- Support staff in reviewing, analyzing, and utilizing the health services delivery data in ChildPlus for the purpose of individualizing services to children.
- Provide leadership and motivation to health services staff. Support in decision making, help mediate conflicts, and provide input to solve problems.
- Provide ongoing coaching and problem solving with staff using regular reflective feedback to ensure strong service delivery and client outcomes.
- In partnership with home visitors, conduct prenatal and post-partum home visits.
- Meet with clients and families in our Medicaid long term programs as requested; review client information, and provide input to client assessments, care plans, and service records.
- Provide nursing consultations to staff as requested; offer clinical insight to improve health outcomes; determine required nursing intervention/activities that may include making telephone calls to clients and/or home visits, follow-up to other health care professionals such as client Primary Care Providers and Home Health Care staff.
- Plan and perform onsite education and training on disease management and health promotion with staff, clients, and client caregivers.
- Follow state and program policies and procedures for nursing referrals for a variety of risk factors including pain management and skin break down.
- Discuss medical/nursing issues and responsibilities with the nurse manager on a regular basis.
- Lead monthly case reviews of medically complex clients with case managers.
- Provide referrals to needed services such as Adult Day Health program, Substance Use Disorder program, medication management, and other support services.
- Perform other duties as assigned.
- Bachelor's or higher degree in Nursing or Registered Nurse (RN) with current Washington State license
- Three years' experience in nursing or community health
- Complete the Washington State Nursing Commission's continuing competency requirements (achieved through active practice, self-assessment and reflection, and continuing nursing education).
- Knowledge of the information and techniques used for well-childcare, prenatal, postpartum care, and communicable diseases.
- Knowledge of or willingness to learn geriatric and/or nursing care for persons with disabilities, wound management, health promotion, and case management in public health and home settings.
- Comfort with public speaking and providing training to non-medically licensed staff.
- Training and experience with public health, or health education.
- Experience with data collection, analysis, and reporting to stakeholders including parents, staff, program management and program leadership.
- Demonstrated ability to communicate effectively, both orally and in writing, with people from a wide range of cultural and economic backgrounds.
- Good interpersonal and communication skills, including the ability to work as part of a team, communicate effectively both orally and in writing, receive and provide feedback, and manage dynamic interchanges in meetings.
- Demonstrated ability to thrive in a culturally diverse workplace dedicated to equity and inclusion.
- Comfort and willingness to work with linguistically and culturally diverse clients and families of all ages and backgrounds. Willingness to comply with established agency performance standards which may include: productivity/personal responsibility, client/customer service orientation, embracing a team player orientation, entrepreneurialism, and maintaining a professional demeanor.
- Willingness to comply with Head Start Standards of Conduct by promoting the unique identity of each child and family; conforming with program confidentiality; following child supervision and guidance policies; and refraining from soliciting or accepting personal gratuities, favors, or anything of significant monetary value from contractors or potential contractors.
- Establish a professional development plan and participate in ongoing professional development.
- Must have reliable transportation and be able to travel independently to Neighborhood House service sites and client homes in King County.
- Excitement, passion, and heart to work with communities served by Neighborhood House.
- Familiarity with and experience implementing Head Start/Early Head Start performance standards preferred.
- Experience in pediatric community health or school health nursing
- Bi-lingual ability.
- Current or former Head Start and Early Head Start parents or any other Neighborhood House program participants receive preference for employment vacancies for which they are qualified.
- Demonstrated prior experience in leadership and conflict resolution. Prior experience in supervision and/or motivation of staff around common goals.
- Registered Nurse in Washington State
- Must pass Neighborhood House background check requirements.
- Position requires Washington State bi-annual criminal background re-check.
- Current Health Exam, Tuberculosis Screening, and record of Measles, Mumps, Rubella (MMR) vaccination or immunity (must be obtained prior to employment).
- Current First Aid Certificate within 30 days of hire.
- Current CPR Certificate within 30 days of hire.
- Sexual Harassment Training within 30 days of hire.
- Child Abuse and Neglect training within 2 weeks of hire.
- Food Handlers certificate within 30 days of employment.
