Location: International Falls, MN

Start Date: Immediate or pending approval

End Date/Duration: 10/12/2026

Shift: 8-hour days, M-F (flexible options like 4-10s available)

Hours/Week: 38 to 40 hours

Employment Type: Permanent placement

License/Certifications: Minnesota Physical Therapist license, BLS within 1 year of hire

Experience: Minimum 1 year in a health-related field, electronic medical records experience (preferably Epic)

Salary: $80,000 – $115,000 annually based on experience

Pay package discussed upon application

Perm placement fee based on salary (20% of annual salary minus 6% TRS vendor admin fee)

What You'll Do

• Provide outpatient and inpatient physical therapy services
• Assist in treatment planning and patient care management
• Document patient progress using Epic EMR system
• Support outpatient, inpatient, and occasionally SNF/home health patients
• Collaborate with healthcare team members to optimize patient outcomes
• Participate in weekend and holiday rotation as scheduled
• Maintain compliance with facility standards and patient care protocols

What You Need

Required:

• Minnesota Physical Therapist license
• BLS certification (within 1 year of hire)
• At least 1 year of relevant experience in healthcare
• Technical proficiency with electronic medical records (preferably Epic)

Preferred:

• Experience across outpatient and inpatient settings
• Strong communication and teamwork skills
