Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments to identify the mental health, substance abuse, and medical needs of clients appropriate to primary care settings.

• Deliver therapeutic services, including individual and group counseling, intake assessments, crisis interventions, treatment planning, and referrals to substance abuse treatment programs when necessary.

• Document all client interactions and treatment interventions electronically, ensuring compliance with relevant regulations and facilitating seamless communication within the care team.

• Provide a full range of liaison services, identifying local resources and community agencies, including medication assistance programs, to support the broader needs of our clients.

• Work collaboratively with Quality Improvement teams to refine and implement effective data collection processes, contributing to the enhancement of service delivery methods.

Your work will focus on individuals dealing with acute and chronic health issues, mental health disorders, as well as families navigating the complexities of hospice and palliative care. Qualifications\n

• A valid state license as a Licensed Clinical Social Worker (LCSW) is required, with ongoing compliance to maintain licensure.

• A Master’s degree in Social Work from an accredited institution is essential.

Compensation and Benefits\n
