Job Summary We are seeking an IRIS Consultant (IC) to partner with community members enrolled in the Wisconsin IRIS Program and the TMG IRIS Consultant Agency. The position is remote, with regular in‑person visits to participants’ homes and community locations to support their goals related to employment, housing, health, safety, community membership, transportation, and lasting relationships. Responsibilities Meet with each IRIS participant at least four times per year, including one mandatory in‑home visit. Provide program orientation and education on rights, responsibilities, legal documentation, and payroll for public dollars. Explore the participant’s life, goals, relationships, community connections, employment interests, and backup support plans. Help participants set personal outcomes and keep those outcomes within the IRIS budget while meeting DHS requirements. Document all orientation, planning, and progress activities in the IRIS data system (WISITs) within 48 business hours of each visit. Research and share community resources and natural supports that align with participant outcomes. Record changes to the plan and data system whenever a participant requests a modification. Prepare budget amendment or one‑time expense paperwork as required. Educate participants on reading and interpreting monthly budget reports and act as liaison between the Fiscal Employer Agency and the IRIS Consultant Agency, including provider billing, receipt tracking, and risk mitigation. Develop trusting, engaged relationships with participants and communicate program changes and compliance effectively. Maintain confidentiality and HIPAA compliance. Collaborate with other ICs and staff to implement participants’ plans. Attend in‑person monthly team meetings and weekly phone check‑ins with the supervisor, plus other related duties as assigned. Required Qualifications At least 2 years of health‑care experience (preferably in care coordination) and at least 1 year serving target groups of the IRIS program or an equivalent combination of education and experience. Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (or four years of commensurate experience if no degree). Valid and unrestricted driver’s license, reliable transportation, and adequate auto insurance for job‑related travel. Ability to build positive work relationships with coworkers, clients, participants, providers, regulatory agencies, and vendors. Demonstrated self‑motivation and the capacity to work independently with minimal supervision. Knowledge of long‑term care programs and principles of self‑determination. Problem‑solving, critical‑thinking, and excellent time‑management and prioritization skills. Ability to manage multiple projects simultaneously and adapt to change. Professional communication, both verbal and written, and proficiency with Microsoft Office and other relevant software. Proactive, detail‑oriented, and comfortable working with diverse populations across varied settings. Knowledge of community resources and the ability to develop professional relationships in challenging situations. Pay Range: $19.84 – $38.69 / HOURLY.
