Description

• Assess patients in either inpatient or outpatient environments and craft personalized treatment plans.
• Implement physical therapy interventions with a focus on improving mobility, strength, balance, and overall function.
• Partner with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal results.
• Educate patients and their families about therapy goals, discharge planning, and necessary home exercises.
• Maintain thorough documentation of evaluations, treatments, and progress in compliance with facility and regulatory standards.
• Engage in care team rounds and interdisciplinary conferences to facilitate coordinated patient care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution.

Certifications

• Active state licensure as a Physical Therapist .
• CPR certification is preferred.
• Specialized certifications (e.g., in neurological or geriatric care) are advantageous but not mandatory.

Skills

• Previous experience is strongly preferred.
