Community Primary Care Physician - Home Visits & Impact

WelbeHealth is seeking a Primary Care Physician to deliver life-changing, community-based care to our most vulnerable seniors in a team-based setting. You will conduct in-person assessments, partner with the interdisciplinary team to craft personalized care plans, and coordinate timely discharge planning for various placements. There is no hospital rounding; instead, you will see about eight patients per day, with regular hours and home visits included in the schedule. #J-18808-Ljbffr WelbeHealth
