Office Location: 349 Southport Cir STE 106, Virginia Beach, VA 23452
Coverage area: Travel required throughout VA Beach, Portsmouth, Norfolk and Chesapeake areas
Schedule: PRN

As a Nurse Practitioner You will:
- Provide compassionate medical care and symptom management
- Perform assessments, order/ interpret diagnostic tests, and develop care plans
- Prescribe medications, manage treatments, and coordinate hospice services
- Conduct and document face-to-face visits for eligibility and recertification
- Collaborate with physicians and IDT team to align ca
