PHYSICAL THERAPIST (PT) - Home Health

Flexible Schedule | Full-Time or Part‑Time
Competitive Hourly Pay (DOE) + Mileage

Practice Physical Therapy, Not Productivity.

SummitWest Care is seeking a compassionate and skilled Physical Therapist to provide one-on-one care throughout Mesa County.

Responsibilities

• Evaluate patients and develop individualized, person-centered plans of care
• Provide skilled physical therapy services in patients' homes
• Improve mobility, strength, balance, and functional independence
• Educate patients and caregivers regarding treatment goals and home exercise programs
• Monitor progress and adjust treatment plans based on patient needs and goals
• Complete accurate, timely clinical documentation
• Collaborate with nurses, therapists, physicians, caregivers, and other members of the care team to achieve optimal outcomes

Qualifications

• Current Colorado Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program
• Valid driver's license, reliable transportation, and auto insurance
• Strong clinical evaluation and treatment skills
• Excellent communication and documentation abilities
• Ability to work independently while collaborating effectively with an interdisciplinary team
• Ability to adapt professionally in a home-based care environment
