- Conduct thorough evaluations in both inpatient and outpatient environments, designing tailored treatment strategies
- Facilitate physical therapy interventions focusing on mobility, strength enhancement, balance, and overall functional improvement
- Engage with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes
- Provide crucial education to patients and their families about therapeutic goals, discharge planning, and essential home exercises
- Meticulously document evaluations, treatments, and patient progress in adherence with institutional and regulatory standards
- Partake in interdisciplinary care team rounds and conferences to ensure the highest standard of coordinated patient care
- Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
- State licensure as a Physical Therapist
- CPR certification preferred
- Additional specialized certifications (e.g., geriatric, neurological) are advantageous but not mandatory
- Prior experience is highly preferred
- Ability to manage complex medical and mobility-related patient cases
- Exceptional communication, interpersonal and teamwork skills
- Familiarity with digital charting and clinical documentation platforms
