Clinical Manager (RN), Home Health, Full Time Adventist Health, Gladstone, OR Full Time Job Summary Manages patient discharges with patients, families, and the healthcare team. Develops home‑care plans for post‑hospital needs. Acts as a liaison with hospitals, skilled nursing homes, board and care homes, outpatient discharge planning services, and physician offices. Arranges home delivery of medical equipment and supplies. Assists with processing patient discharges. Provides support to department leadership for recruitment, selection, training, scheduling, and performance management. Supervises and directs activities of assigned personnel. Job Requirements Education: Bachelor’s Degree in nursing or equivalent; Master’s Degree preferred. Experience: 5 years in home‑care nursing (preferred); 1 year of leadership experience (preferred); 1 year of hospital case management, including discharge planning (preferred). Licensure: Registered Nurse (RN) or Physical Therapist (PT) in state of practice required; Public Health Nurse licensure preferred. CPR or BLS certification preferred. Valid driver’s license required, and applicant must be at least 21 years old. Essential Functions Participate in discharge‑planning discussions with patient, care team, and families; coordinate continuity of care. Conduct daily rounds on nursing units to review home‑care discharge plans and monitor admissions. Compile monthly statistics and provide chart information to support Home Health and hospice standards of care. Report patient assessment and evaluation to physician/Team Leader/Manager of Clinical Services. Assist with orientation of new employees and provide in‑service education to hospital and medical staff. Assess referrals and continuation of services from community agencies; assist with marketing, business, and strategic planning for the home‑care program. Evaluate medical supplies and equipment needed for patient home care; order and arrange delivery. Recruit, select, and train staff; conduct performance evaluations and coaching; address staff complaints and scheduling issues. Attend appropriate hospital committee meetings and perform other assigned duties as required.
