Role: RN Case Manager providing compassionate, patient-centered hospice care in Effingham and surrounding areas. Responsibilities Include Conducting comprehensive assessments to identify patients' evolving needs Developing and adjusting personalized care plans with interdisciplinary teams Administering medications and treatments as ordered Maintaining accurate and meaningful clinical documentation Acting as a communication link among families, physicians, and community health partners Educating patients and caregivers to promote confidence and support Participating in on-call rotations as per team policy Qualifications & Skills Active RN license in the state of practice Graduate of an accredited nursing program Preferably 1+ year experience in hospice, home health, or related fields Strong clinical judgment and ability to work independently Excellent communication skills for collaboration with care teams and families Compassionate approach to end-of-life care This role offers competitive pay, comprehensive benefits, company-provided equipment, and opportunities to make a meaningful difference in patients' lives. #J-18808-Ljbffr Vetted Solutions
