JOB DUTIES AND RESPONSIBILITIES Case management services that include linkage, coordination of care with community partners, family and clinician. Family Rehabilitation Provide individual rehabilitative services Document services in electronic mental health records Maintain professional trauma-informed and caring interaction with clients and their families This position is field base and services are provided both in the office and at clients homes. QUALIFICATIONS/REQUIREMENTS Must have a BA or BS in psychology, social work or other related field Experience in the mental health field is a plus Knowledge and understanding of trauma-informed care and resiliency practices is a plus Monday through Friday 8:30-5pm Must have a valid Driver’s License & car insurance Bi-lingual is a plus.
