MDS Coordinator LPN or RN

Job Details: Level: Experienced, Job Location: CCLA 9 Riverview Jefferson - Detroit, MI, Salary Range: Undisclosed, Job Shift: Day, Riverview Jefferson is looking for an experienced MDS Coordinator LPN or RN to join the team! The MDS Coordinatorprovides assistancein Resident care planning activities, including the MDS and MMQ Process Serves as a resource person for resident care planning activities. Completes MMQ and turnaround documentsin a timely mannerand communicate as needed withappropriate statedepartments. Ensures that the quality and appropriateness of resident caremeetsor exceeds company and industry standards and ensures that all nursing servicesare in compliance withstate, federal, legal, regulatory, accreditation, and reimbursement guidelines. Supportsprogram development activities. Attends andparticipatesin department/facility meetings, asrequired. Accesses continuing education opportunities appropriate to responsibilities of the job. Promotesoptimal communication among program staff members, facility staff, referral sources, physicians, and residents. Appropriatelyaccessesthe resources of the company’s regional offices for consultation and program development support.Seeks outexternal resources through conferences, workshops, etc.as necessary. Routinely shares personal knowledge with staff, other managers, and facility personnel. Demonstrates flexibility in adjusting to change, resident needs, and customer expectations; meets company standards relating to deadlines and attendance. Established andmaintainsexemplary standards for department appearance and staff presentment; ensures compliance with company and facility policies and procedures. Adheres to all safety and risk management protocols and programs. Participates in direct and indirect resident care activities aswarranted. Participates in quality improvement activities, which reducecostand increase efficiency through improved systems and processes. Exhibits courtesy, compassion, and respect to residents, families, visitors, physicians, administrators, and coworkers. Performs other duties or functions as directed. Qualifications: Working knowledge of MMD/MMQ rules and regulations.Previousexperience required. Graduate of an accredited school of nursing. /Valid license as an LPNin Michigan REQUIRED. Excellent Resident Assessment Skills Knowledge of regulatory standards and compliance requirements. Freedom from illegal use of drugs. Freedom from use of and effects of use of drugs and alcohol in the workplace. Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a healthcare relatedsetting are ineligible for employment in this position. Working knowledge of personal computer and software applications used in job functions (word processing, graphics, databases, spreadsheets, etc.). Strong organizational and analytical skills; oral and written communication skills. Demonstrates ability in independent functioning and strong leadership.
