This is an autonomous field-based role that focuses heavily on OASIS assessments, admissions, case management, and skilled therapy services. The ideal candidate is comfortable working independently, managing a caseload across a designated territory, and coordinating care with physicians and interdisciplinary team members. Responsibilities Complete Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and follow-up visits Develop and manage individualized physical therapy plans of care Perform OASIS documentation accurately and timely Coordinate care with physicians, nurses, social workers, and other healthcare professionals Travel within approximately a 50-mile radius to provide home-based patient care Maintain compliance with home health regulations and agency standards Requirements Active Washington State Physical Therapist License Current BLS Certification Valid Driver's License Reliable Transportation Current Auto Insurance meeting state requirements Home Health experience required/preferred OASIS experience strongly preferred HCHB experience preferred Submission Requirements Resume Active License Why This Assignment?
