Responsibilities

• Deliver physical therapy services to patients in a home health/home-based care context, focusing on mobility, strength, balance, functional independence, and pain management.
• Assess patient needs, develop and implement individualized treatment plans, and adjust interventions based on progress and goals.
• Document evaluations, treatment plans, progress notes, and discharge summaries in accordance with regulatory and billing procedures.
• Educate patients and caregivers on home exercise programs, safety strategies, and use of assistive devices; provide clear instructions for ongoing care.
• Collaborate with an interdisciplinary team to coordinate care, monitor patient safety, and plan transitions or discharges.

Requirements

• Ability to work in a travel assignment and provide therapy in a home-based care setting.
• Willingness to travel to patient locations within the assignment area and adhere to a day shift schedule (40 hours/week).
• Ability to review, follow, and upload required documentation in accordance with billing and credentialing procedures.
• Demonstrated ability to work independently as a physical therapist while maintaining professional standards and patient privacy.
