Description

American Traveler is hiring a Home Health Occupational Therapist with a minimum of 1 year of experience to provide skilled home care to a diverse adult patient population.

Details

• Home health setting providing skilled visits to patients covered by Medicare, Medicare Advantage, and commercial insurance
• Diverse patient population including med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, neuro, trauma, and amputee cases
• 6-8 home visits per day with a minimum of 6 points per day
• Day shift schedule, 5x8 hours, 8:00 AM-5:00 PM
• 1-2 weekends required per contract
• Self-scheduling of visits and hours based on office needs
• Use of laptop, MiFi, and portable printer for documentation
• Charting completed via BayCare Forms, BayCare Scheduling, and Ace Timesheet systems

Requirements

• Active Florida or Compact OT license required if applicable
• BLS certification required
• Minimum 1 year of experience required, 2 years preferred
• Home health experience required
• OASIS training preferred
• Must function as an independent practitioner
• Cerner experience preferred
• Valid driver's license and personal vehicle required for home visits
• 1:7 patient ratio experience
• Experience with prosthetics, orthotics, and splinting techniques preferred
• Experience with adult, amputee, CVA, Parkinson's, and neurological rehabilitation populations preferred

Additional Information

• Perform assessments, evaluations, reassessments, discharge planning, and OASIS documentation
• Provide home exercise programs, patient/family education, and safety training
• Utilize adaptive equipment, wheelchair evaluations, and home assessments as part of care plan
• Must be COVID vaccinated or have exemption on file
• Flu shot or declination required, with mandatory masking if declined
• Solid color scrubs required
• Former BayCare employees must wait at least 12 months before returning as a traveler
• RTO is not permitted in this contract
• Candidates must reside more than 100 miles from the facility to qualify for travel rate
