NP or PA, Senior Community Care (Lane County)

Optum Home & Community Care (part of Optum) is seeking a full‑time Senior Community Care (SCC) clinician (Nurse Practitioner or Physician Assistant) to provide integrated care at home, nursing homes, and assisted living facilities in Lane County, Oregon. Position Overview Full‑time SCC clinician (NP or PA) traveling to facilities throughout Lane County, Oregon. Monday‑Friday 8:00am – 5:00pm. No on‑call, no weekends, no holidays. 23days paid time off plus 9 paid holidays. Primary Responsibilities Conduct comprehensive assessments. Manage members through acute medical situations or chronic condition exacerbations, including testing, diagnostics, and interventions. Manage members during short‑term sub‑acute rehabilitation episodes. Ensure accurate and complete documentation, including ICD‑10 conditions. Communicate and collaborate with the interdisciplinary care team. Conduct advanced illness and advanced care planning conversations. Provide patients and caregivers with counseling and education. Required Qualifications Nurse Practitioner Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program. Active, unrestricted NP license in the state of residence (or ability to obtain by start date). Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center in one of the following areas: Family NP, Adult NP, Gerontology NP, Adult‑Gerontology Acute Care NP, Adult‑Gerontology Primary Care NP. Physician Assistant Graduate of an accredited Physician Assistant degree program. Active, unrestricted PA license in the state of residence (or ability to obtain by start date). Board certified by the National Commission on Certification of Physician Assistants (NCCPA). Current, active DEA licensure or prescriptive authority (or ability to obtain post‑hire, per state regulations). Ability to gain a collaborative practice agreement, if applicable in your state. Ability to complete physical requirements of the job, including moving a 30‑pound bag, navigating stairs, and remaining in a stationary position during assessment. Driver’s license and reliable transportation. Preferred Qualifications 1+ year clinical experience in long‑term care or similar setting. Experience meeting needs of patients with complex behavioral, social, and/or functional requirements. Understanding of geriatrics and chronic illness. Understanding of advanced illness and end‑of‑life discussions. Proficient computer skills and ability to document in electronic medical records. Compensation and Benefits Salary ranges from $91,700 to $163,700 annually based on full‑time employment.
