If you are passionate about delivering exceptional patient care and are committed to making a positive impact, we encourage you to apply for this exciting opportunity.Responsibilities:Provide nursing care within the scope of licensure for assigned patients according to established nursing standards and hospital policies and proceduresAdminister medication, perform treatments, and patient care responsibilities as prescribedConsult with and keep MD/NP informed of activities, symptoms, reactions, and changes in the patient conditionProvide appropriate pain management techniques and educate patients and families regarding pain managementAdminister medication following the 10 rights of medication administration within scope of practiceAssist with the rehabilitation and treatment of patients in accordance with the medical plan of careUpdate and revise plan of care under the supervision of MD/NPAssist physicians with examinations, treatments, and special proceduresPerform services requiring technical and manual skills as directed by MD/NPTreat patients and their families with respect and dignityIdentify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their familiesInteract professionally with patient/family and involve patient/family in the formation of the plan of careAssist with admitting, discharging, and transferring of patientsIdentify and address psychosocial needs of patients and family; communicate with Social Services/Discharge Planner regarding both in-hospital and post-discharge needsMedication Administration & ManagementAdminister medications per physician/NP ordersManage IV, SQ, SL, rectal, transdermal, and other hospice-related medication routesTitrate comfort medications within ordered parametersMonitor for side effects, adverse reactions, and effectivenessEnsure timely availability of emergency comfort medicationsCoordinate pharmacy needs with hospice providerCoordination with Hospice Interdisciplinary Team (IDT)Communicate patient status changes to hospice physician/medical directorCollaborate with: Hospice aides, Social workers, Chaplains, Nurse practitioners, Facility nursing staffParticipate in hospice IDT discussions and care planningCoordinate transfers, discharge planning, or level-of-care changes if symptoms stabilizeDocumentation & Regulatory ComplianceDocument assessments, interventions, and patient response thoroughlySupport documentation demonstrating: Need for continued GIP level of care, Uncontrolled symptom burden, Frequent nursing interventionsComplete required hospice and SNF documentation timelyEnsure compliance with: CMS hospice regulations, State nursing regulations, Facility policies, Hospice Conditions of ParticipationFamily & Patient SupportEducate family regarding: Disease progression, Dying process, Medication purpose/effects, Comfort-focused goalsProvide emotional support during crisis situationsAssist with difficult decision-making discussionsSupport cultural and spiritual preferencesOffer bereavement-related support and referralsSkilled Nursing InterventionsFrequent reassessment and bedside monitoringWound and skin care as appropriateFoley catheter managementOxygen therapy managementSuctioning and secretion managementFall prevention and safety monitoringPositioning and pressure injury preventionInfection prevention practicesRequirements:RNAbility to work varying shifts, including days, nights, and weekendsWork Environment:The Bartlett Skilled Nursing and Assisted Living is a locally owned and operated organization that values its commitment to quality care and community involvement.
